For the purpose of requiring STEINMED's Ad-Hoc services, where by a request for the supply of HCRW services, or the outright purchase and supply of Medical, Health-Care and Consumable Products, STEINMED has below supplied the application and product order form needed to do so.
Since STEINMED does not accept COD payments, or payment by Cheque collection, the debit order authorization form has been included.

It is the primary business objective of SteinMed to convert all of its account base customers into fixed service frequency contracts which will limit the service frequency to not longer than 30 days apart.
For those clients wishing to move over on to a contractual service schedule with a pre-determined, fixed service frequency i.e.
